Call us for an appointment and experience the latest cardiovascular treatments with an emphasis on patient-focused care. Same-day scheduling is often available. Most insurance plans are accepted.
New and Existing patients may also log in to our E-Care Portal to schedule an appointment, fill in forms, email us etc online.
What to Bring to Your Appointment
If you are a first-time patient, please arrive early to complete patient registration forms. Please bring the following items with you:
- Insurance card(s)
- Referral forms from another physician (if required by your insurance plan)
- List of all prescription and over-the-counter medications that you are currently taking
- Your medical and surgical history information
- X-rays, CT films, ultrasound tapes, etc. from previous medical exams
- Any required co-payment
Insurance and Payment Information
Cardiac Care is a provider for Medicare and most major insurance plans. Since we provide insurance billing, we encourage all patients to confirm their coverage prior to making an appointment.
Co-payments are due at the time of the appointment. If the insurance company does not pay the full balance, or denies the claim, then any unpaid balance is due from the patient within 30 days following the appointment.
For those patients without insurance, Cardiac Care offers on-site financial counseling.
